As part of Middlesex Health’s commitment to creating a fully inclusive workplace, they have enlisted Diversity Science to conduct an independent survey of the organizational equity and inclusion climate. Middlesex Health leadership wants to hear from you about your perspectives and experiences so that they can continue to improve and be the best place to work.
Diversity Science is a mission-driven, public-benefit organization that translates the latest research into practical and useful tools to create diverse, equitable, and inclusive organizations. Employee engagement surveys are very important in telling an organization how employees feel. This survey will give insight into why you feel the way you do. Diversity Science will use the results to give Middlesex Health specific, actionable recommendations.
It takes most people 10-15 minutes or less to complete the survey. There are places to write in your thoughts and opinions, so the amount of time it takes could be longer if you want to contribute written comments to the assessment.
No. However, we encourage you to complete the survey at one time. You can log out and log back in as many times as you need, but you must use the link in the email you received, so please save that link. If you have any trouble, email [email protected].
This survey is completely confidential. We have multiple safeguards in place to ensure that your individual responses will never be linked to your name or other identifying information.
· The online questionnaire is being hosted on a secure external website. This is outside of and unconnected to any company or organization’s intranet and computer network.
· Answers are never linked to your name or any identifying information.
· Any write-in answers will be reviewed carefully, de-identified, and only shared if possible to do so in a way that ensures complete anonymity.
· Diversity Science will summarize the results in a statistical format and report results in aggregate only.
· No one at Middlesex Health can get access to the dataset and try to identify a respondent.
Yes, filling out the survey is entirely voluntary. However, your experiences and opinions are essential to the success of this project!
While we hope you will participate, no one will know whether you participate or not. To ensure responses are confidential, we use an automated reminder system, which means you may continue to get automated reminders until the survey closes.
It is entirely your choice which questions you answer. You can skip any question or any part of the questionnaire and just answer the questions you want to. However, we will gain a more accurate understanding of equity and inclusion at Middlesex Health if we get complete feedback from everyone.
